Personal Health Budget

We work with different local authorities on Personal Health Budget (PHB). PHB is a type of government funding to support an individual’s assessed health and wellbeing needs. It is planned and agreed between the individual and the local Clinical Commissioning Group.

The client’s health and wellbeing needs will be set out in a personal centred care and support plan which is jointly developed by the health care professional and the client.

How our Home Care services works

Once a service user is referred to our service, we will contact the service user to arrange a comprehensive assessment of their social care needs, lifestyle and medical conditions and the support required to enable them to continue living independently at home.

Following assessment, we will develop a support and wellness plan individually tailored and responsive to specifically identified needs, problems and goals of the person. Our focus is on providing a personalised service and maintaining service users independence, continue to live according to their habits and preferences, and maintain existing social and community ties.

We are highly responsive to the varied needs of our service users, their families and the communities we serve. We employ competent, caring, and well-trained individuals.
