Cognithan provides services for adults who might otherwise need ongoing care in a hospital setting offering real alternatives, enabling adults to engage in rehabilitation within a community environment. Our care model stems from our extensive experience in developing and achieving positive outcomes for those who need residential and community-based rehabilitation. Our programmes provide highly individualised, recovery-based approaches to drawing on service users’ strengths and addressing their needs.

Many individuals who use our services have multiple and complex care needs, including those with mental health and learning disabilities.

We take a personal approach to the care and services provided and our care teams are committed to creating space and a therapeutic environment that promotes the high quality care to individuals. This is achieved through promoting enabling independence, preserving dignity, championing freedom of choice and protecting their privacy.

Working closely with health, social care and criminal justice services we have jointly developed bespoke packages of care with commissioners and statutory providers across London and the South East.
