Making A Referral

Referrals to our Services are welcome from a wide range of health and social care professionals, including:

  • Primary Care (e.g. GPs)
  • Community Mental Health Teams
  • Hospital In-Patient Wards

Typical referrers include GPs, Care Coordinators, Social Workers, and Psychiatrists. We take time to understand each individual’s unique needs to ensure they are matched with the most appropriate service and support.
